I went to PP for the first time on 9/12/2014, there were 3 people in the waiting room at the time. I approached the desk and quietly told [redacted] that I was there in need of the emergency contraceptive pill. During my visit I was called up to the front desk at least 5 times to fill out various paperwork and 3 out of the 5 times [redacted] in a normal voice tone asked if I had ever used emergency contraceptive before and then went on to tell me during another trip to the desk that a staff member from the back would be calling me to a room to go over and supply me with the pills. During this time there were up to 5 people in the waiting room. I did not feel that the reason I was at PP needed to be discussed at the desk nor at a volume that other patients and their family members could hear.

Remarks: Dispatched to this location M5 for 30 y/o female patient hemorrhaging post abortion. Find same, clinic staff with patient attempting IV access and controlling hemorraging. Staff physician reports an approximate 2 liter loss. Pt. AAOx’s 3, MAE’s well with nl CMS. M5 on scene, assist with IV access, and movement of patient from clinic exam table to cot and to ambulance. M5 transporting, E8 clear and returning to quarters.

Staff
The laboratory staff failed to monitor temperatures in the laboratory. Specimens and reagents need to be kept at a certain temperature to prevent them from being compromised. Fluctuating or too warm temperatures can prevent proper results.
The laboratory staff didn’t follow the proper procedures for testing samples. Lab technicians didn’t follow manufacturer’s instructions while conducting tests with laboratory equipment.
The laboratory director failed to sign off on documentation of test results.
In another inspection, the facility was found not to have procedures for evaluating the competence of laboratory staff who were conducting tests.
Patients
Four patients who were tested for Rh incompatibility were listed as Rh negative in one set of paperwork and Rh positive in another. Rh negative patients need a RhoGAM shot to protect future pregnancies. Confusion over Rh status can lead to patients who need the shot not receiving it.
The laboratory director failed to review or evaluate the documentation about corrective action for this incident.
